A month or two before you move:

List the items that you really want to bring with you and start throwing out, donating or give away the things you don’t want.
Go to the post office and fill out a moving card, this way all of your mail will be redirected in case you’ve missed telling a few companies or people you know about the move.
Get all of your medical, dental and academic records if you are moving out of the city or having to get a new Dr. or Dentist in your new area.
Consider statutory holidays that might interfere with any services you may need on that day.

A month or less ahead of time:

Call the utilities and phone company to make arrangements for the new home and the shut off dates of your old home.
Confirm with any services you may have booked to help you move that you are on their schedule
